Sungkwang Postick Swap Liquid (Povidin iodine)
1. Do not administer to the following:
1) Patients with hypersensitivity to this medication’s ingredients
2) Patients with abnormal thyroid function (especially nodular thyroidoma, fatty thyroidoma, Hashimoto thyroiditis, etc.)
3) Patients with renal failure
4) Newborn babies and infants under 6 months
5) Herpetic dermatitis patients
6) Before and after radioactive iodine treatment
2. Be careful when administering to the following:
1) Serious patients (burned patients, etc.)
2) Patients with history of thyroid disease
3) Patients with history of renal failure
3. Side effects
1) Hypersensitivity: In rare cases, symptoms of hypersensitivity (poor breathing, premature redness, hives, etc.) and iodine (iododerma) may appear, so stop using it in such cases.
2) Skin: In the case of severe pain, itching, irritation, rash, redness, dermatitis, rare fever, skin ulcer, contact dermatitis, and skin discoloration, stop using the medicine.
3) Electrolyte, Metabolism: As in the case of burn treatment, osmotic pressure disorder and metabolic acidity of electrolyte and serum can occur when administered in large amounts.
4) Kidney: As in the case of burn therapy, large doses can cause kidney failure.
5) Blood: Long-term use may result in changes in hypothalamus, pituitary, thyroid, and axial functions and increased secretion of iodine due to reversible reactions. In addition, serum thyroid stimulation hormone concentrations may rise slightly and T₄ and T₃ reduction may occur.
6) Mass administration: There are reports of a temporary increase in serum iodine binding protein in some patients due to mass or prolonged contact. Repeated administration of this drug to a wide range of injuries or burns can lead to several side effects (metallic flavor, increased saliva secretion, fever sensation and pain in the throat and mouth, eye irritation and edema, skin reaction, upset stomach, diarrhea and pulmonary edema).
7) Others: If this drug is used excessively for disinfection, it is iodotoxic.
4. General cautions
1) When used just before delivery, wash the infant's eyes as soon as possible and prevent attachment by bathing.
2) It is noted that treatment by this drug is an archival treatment, and surgical therapy is considered if it does not show any signs of improving the symptoms.
3) When using this medicine for patients with extensive burns, sufficient hydration shall be provided. Acid, basal metabolism and kidney functions shall be observed and the drug shall be discontinued in the event of metabolic acidosis or dysfunction (excluding cleaning fluids, cleansing fluids/7.5%).
4) If this drug is used for ulcers caused by burns, it is used only for ulcers caused by secondary damage after burns and other appropriate treatments are considered for new burns (excluding cleaning fluids, cleansing fluids/7.5%).
5) Because this drug can affect thyroid diagnosis using scintillation, administration of this drug and scintillation investigation are conducted at intervals of one to two weeks. It is also important to note that thyroid function can change reversibly during administration of the drug.
6) Since this medicine contains iodine content, pay attention to abnormal thyroid function during mass administration and long-term use.
7) In case of infection (peritoneal inflammation, myelitis, etc.), clean it with saline solution before using this medicine. There is a risk of endotoxin release from bacteria due to contact with the drug.
8) Because shock (anaphylaxis) can occur rarely, if severe symptoms such as itching, edema, heartburn, etc. suddenly appear after use, such as pigmentation, cold hands, cold sweat and exhalation, seek medical attention immediately.
5. Interaction
1) Patients who are undergoing lithium treatment should avoid regular use of this medicine. When used in combination with lithium therapy, hyperthyroidism effects can occur.
2) The sterilization effects of chlorhexidine, sulfadiazine, silver compounds, alkali, and mercury may be reduced when combined with the contents of the drug, which can be seen through discoloration.
3) The effects of enzymes are lost when combined with the wound treatment agent of enzyme inhibitors.
4) This medicine should not be used with hydrogen peroxide.
6. Administration to pregnant and lactating women
1) Do not use it regularly, for a long period of time, or extensively for pregnant or likely pregnant women and lactating women.
2) Absorbed iodine is transferred to milk through the placenta barrier. Absorbed iodine can affect the thyroid function of the fetus or newborn baby or cause thyroidoma.
Therefore, it is recommended to check thyroid function in newborns or infants.
7. Effect on clinical examination values
1) If iodine is absorbed through healthy or damaged skin, it will interfere with the thyroid function test.
8. Treatment of excessive dosing
1) Symptoms
- If a large amount of iodine is taken orally, the following symptoms may occur:
- Upper abdominal pain, vomiting and diarrhea, polydipsia, abdominal cramps
- Excessive tension, tachycardia, cyanosis, headache, dizziness, delusion, despondency and coma
- Browning of the mouth mucous membrane, rhinitis, bronchitis, gastrointestinal inflammation, hemorrhaging mucous membrane damage, gastroedema, renal hemorrhage, anurology, and fatal vascular motility failure can occur.
- Excessive iodine may indicate thyroid nodules, hypothyroidism or hyperthyroidism.
2) Treatment
- It is recommended to eat foods containing starch and protein for detoxification. If necessary, stomach wash with 5% sodium sulfate should be performed for every 3 hours or 10ml of 10% sodium sulfate should be injected.
- If ingested in large quantities intentionally or accidentally, pay attention to the electrolyte equilibrium, renal and thyroid functions, and perform allopathy and supportive therapy.
9. Application precautions
1) It shall not be used for ophthalmology or intake. Be careful not to let the undiluted solution get into your eyes, and if it gets into your eyes, wash it immediately with clean water.
2) Do not mix it with other formulations.
3) Wash and sterilize the affected area. Use distilled water or sterile purified water (no tap water or purified water) as a diluent when used for severe frosting.
4) If the medicine gets on the clothing, etc., it is easily washed with water if washed immediately. In the case of synthetic fibers, it can be erased with ammonia water or sodium sulfate solution.
5) Because soap weakens the sterilizing effect of this medicine, wash it off clean before using.
6) Since it becomes electrically insulating when dried, be careful not to let this medicine get between the antipolar plate and the skin when using an electrical scalpel.
10. Storage precautions
1) Store it away from direct sunlight.
2) When metallic containers are used for storage, they may be corroded by oxidation characteristics of iodine. Synthetic resin containers can be used, but reversible discoloration may occur.
11. Others
1) There are reports of an increase in inorganic iodine and total iodine levels in the blood due to use in the vagina.
